At Jewish Care, we believe that all people are created equal and deserve the freedom to participate within the community. Our focus is on helping every person with a disability to lead an inclusive and rewarding life. Jewish Care’s individual and family services provide flexible, culturally sensitive options for people to achieve this.

Futures for Young Adults


When young people with disabilities leave school, Jewish Care can support their transition. Jewish Care develops and implements person-centred plans based on the individual’s unique interests, skills and aspirations to support them to explore options and pathways to adult education, training and employment.

Outreach Support

Jewish Care’s Outreach Support Program delivers services and activities in the home, supporting people with disabilities to live independently in their own home and in the community. This may include shopping, budgeting or learning a new skill to enable people to continue living independently and to easily participate within their community.

Outreach support services include help with:

  • accessing community recreation,leisure and cultural activities
  • learning how to use local facilities such as banks, shops and public transport
  • using health care services such as dentists, home nursing and other therapies
  • developing household management skills such as cooking, laundry and budgeting
  • getting around using mobility equipment for people with vision impairment
  • dealing with emotional responses and difficulties
  • sourcing employment opportunities

Support may be short term, long term, or intermittent. Our services ensure that people have choice and can participate within the community.
