Individual Support Packages
An Individual Support Package (ISP) is a government funding allocated by the Department of Human Services (DHS) to assist people to meet their disability support needs and achieve personal goals.
Case management and care coordination can be purchased through ISP funding to help you negotiate the range of support offerings available and select the types and hours of support that best fit your lifestyle.
Our care support services are tailored to meet the individual needs of our clients – older people, all people with a disability, and their families to maximize their personal choices at home and in the community and independence in building fulfilling lives.
What Individual Support Packages can be used for
Individual Support Packages may assist people with a disability to continue living in the community by providing a range of supports to:
- maintain independence
- keep living in their own home
- move to more independent living arrangements
- learn new skills
- participate in the local community.
Case managers assist in locating services and funds for equipment, as well as guide people through referral processes with other health and community organisations.
